Why do people who use "gay" as an insult try to tell me they don't mean to offend gays?

I hate this term so much. People get mad when you tell them not to use it because they don't understand its significance. And if they don't think that they're insulting gays it because gays use it thoughtlessly as well or no one made them realize what it really means.
